Dear Parents,

We are happy to announce that we will again be offering the services of the “Brace Bus” to you and your children for the 2007-2008 school year.  The Brace Bus will start September 4, 2007 and will serve the following schools:

• Easton Area School District
• Nazareth Area School District
• Wilson Area School District
• Easton Catholic Elementary
• Holy Family Elementary
• St. Jane’s Elementary
• Notre Dame High School

In order for this service to operate smoothly, we ask that the following guidelines be followed.

1. Patient’s legal guardian must complete the attached “Brace Bus” forms and return both forms to our office in advance of this service being utilized.  Please do not mail these forms directly to your school.  These forms can be returned by:

2. Brace Bus riders must be scheduled in such a manner as to not interfere with other riders.  This will be done on a first come, first serve basis.

3. If a patient “no shows” two times for a brace bus appointment, they will not be allowed to ride the bus for the remainder of the school year. 

4. A note will need to be written by the parent or guardian allowing the patient to be dismissed from class 20 minutes before their scheduled appointment time.  This will allow our driver adequate transportation time.  Patients who do not have a note will not be able to be transported to their appointment and will be considered a “no show”.

5. The “Brace Bus” will run between the hours of 9:00am & 12:00pm.  This is necessary to avoid school traffic.

6. Children riding the “Brace Bus” for appointments ending after 12:00pm must make arrangements to be picked up at our office after their appointment.  We cannot transport children to or from home.

7. If there is a possibility that your child may miss their scheduled school lunch, please be sure to pack a lunch for them to eat at our office.

8. In the event of inclement weather, we may suspend “Brace Bus” service for the day.  Please ensure that we have a correct daytime phone number where you can be reached in case we need to contact you.

9. We will mail you a slip summarizing the day’s appointment.

10. Your child will be scheduled for their next appointment with us.  Please call the office to reschedule if this is not convenient.

11. If your child will not be attending school the day of their brace bus appointment please notify us as soon as possible to cancel this service for that day.
